JOB SUMMARY
The Case Management Liaison conducts pre-admission/readmission assessment visits and reviews to determine appropriateness for short or long-term placement in the skilled nursing facility in accordance with regulatory guidelines. Interfaces with physicians and hospital personnel to ensure a seamless transition of care, coordination of equipment, authorization by insurance. Pre-screens for utilization issues and confers with IDC team/Medical Director/appropriate department(s) regarding specific cases.

QUALIFICATIONS/REQUIREMENTS
Registered Nurse or Masters in Social Worker required. BSN, LSW and/or certified case manager preferred.

Minimum of two years related experience, preferably in acute care, skilled nursing famility or home care.
