Cambridge Medical Center, located just north of the Twin Cities metro area in Cambridge, Minn., provides comprehensive health care services in both a hospital and large multi-specialty clinic. With 800 employees, 66 physicians and 86 licensed beds, Cambridge Medical Center serves medical, surgical, specialty care, high-risk neo-natal care, substance abuse, mental health and wound care patients. Outpatient care includes same day clinic and surgery, infusion therapy, eye clinic, optical shop and retail pharmacy. Cambridge Medical Center is also a certified Virginia Piper Cancer Institute® and Courage Kenny Rehabilitation Institute location.
Assessment of patient, including family situation, physical limitations, vulnerability, mental status, special cares, financial situation, communication ability and services receiving or needed
Counseling: Short-term casework counseling related to health care and coping with changes
Assistance: Arranging for team/family conferences, working with patient and family to establish realistic care plans, utilizing community resources to enable patient to follow through with care plan
Establishes working rapport with physicians, nurses, and other staff and facilitates communication between staff and patient/family
Works as a team member with staff to assist patient to return to optimal health situation
Participates in appropriate hospital committees and task forces to ensure quality patient care, and planning for the future of hospital programming
Participates in appropriate community task forces, committees and projects to represent the hospital's philosophy and commitment to the community
Documents notes and plan of care on each patient assessed
Enters summaries of contacts with patient/family, agencies, friends, institutions and staff into Excellian computer system
Completes administrative duties for Social Service office
Continuously works on projects and ideas to improve the quality of social services provided to patients, families, staff and community
Minimum of a BS degree from approved school of social work
Two years of casework in a health institution, child welfare agency, psychiatric clinic or family counseling service is preferred
Maturity to be able to handle stressful interpersonal situations
Excellent communication skills
Broadminded acceptance of people from different lifestyles, religions, beliefs
Ability to work as a team member
Allegiance to hospital’s philosophies
Ability to be self directed
Computer skills for data entry and other specific information
Currently licensed to practice social work in the State of Minnesota
This is a casual position work hours 0800-1630 with Weekend rotation per unit need.
