We are seeking full-time Hospice Nurses to join our team and earn a $2,500 sign-on bonus! This is a Monday thru Friday Day Shift w/On Call. We are staffing our Northern (Schaumburg, Streamwood, Bloomingdale), our Central (Lombard/Elmhurst) and Southern (Glen Ellyn, Orland Park) suburban locations. Come join our growing team!

Position Description:

Our Hospice Nurses are responsible for the delivery, coordination and monitoring of appropriate services for the patient and participation in the development/implementation of the patient care plan. Additional duties include educating the patient/caregiver on hospice services and philosophy, assessing the patient for hospice appropriateness, initiating hospice admission including a comprehensive physical/emotional/psychosocial assessment, coordinating optimal care and use of resources with other hospice staff and assisting the patient/family in coping with the dying process.

You will see patients in their homes and at our Lexington Facilities.

Position Requirements:

  • Must possess at least one (1) year of clinical RN experience.
  • Current State of Illinois (RN) in good standing.
  • Current CPR for Healthcare providers
  • Current valid driver’s license and proof of auto insurance.
  • Able to use computer for patient’s EMR.
  • Willing to participate in an “On Call” schedule.
  • Must be able to effectively communicate in English, both written and oral.

PREFERRED:

  • Previous Home Health and/or Hospice experience, highly preferred.
