Post Acute Rehab Services

Job Duties: Rehab Clinical Coordinator to work with Rehab Manger and interdisciplinary post acute rehab team for: Program Development, Training, Education, Clinical and Administrative Management, Outcomes and Performance reporting, Regulatory Compliance tracking, Process Improvement, Workflow development/management, Staffing/Scheduling, Performance Reviews, and Interdisciplinary planning/communication. Additional duties to include patient assessment/evaluation, patient treatment, and associated documentation, discharge planning, and team rounding.

Full Time (40 hours)


Every 3rd weekend is required

Work requires the knowledge of theories, principles, and concepts typically acquired through completion of a Bachelor's degree in Physical Therapy or Occupational Therapy or closely related field.

Requires licensure as a Physical, Occupational Therapist, or Speech Language Pathologist. Valid AHA Healthcare Provider CPR card is required.

Two years of previous experience. Previous experience in a post acute setting is preferred.

Possible exposure to communicable diseases and hazardous materials with little likelihood of harm if established health precautions are followed
