Home Health Aide is non-professional work in providing home health services to homebound patients. Employees in this class provide basic personal and physical health care services to patients in their homes.
Responsibilities
- Documents care provided and submits it within 24 hours, in order to meet Federal, State, and Agency requirements as evidenced by medical record review.
- Weekly productivity goal for patient care visits in accordance with the established Agency Productivity Standards as evidenced by supervisory review. Adjusted productivity will be documented based on branch patient census.
- Observes patient’s condition including physical, environmental, social and psychological needs. Reports patient problems or issues to nurse/supervisor per documentation and conference as specified in agency policy evidenced by medical record review and supervisory visits.
Qualifications
- High School Diploma or GED.
OR
- Completion of the eleventh grade including one year of experience in a health care facility or providing health care to homebound
- patients.
OR
- Completion of the tenth grade including two years of experience in a health care facility or providing health care to homebound patients.
