Madonna Living Community, a BHS community offering skilled nursing, outpatient rehab, transitional care, memory care, assisted and independent living services, is currently seeking a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) in our skilled nursing department.
This is a part-time position working 2nd shift, including every other weekend and scheduled holidays.
The RN is responsible for developing and implementing appropriate plan of care based on the residents’ needs, evaluating and making the necessary changes to the plan of care. Plans and provides education to other health care team members, residents, family, and caregivers on information regarding the care plan for the residents. Demonstrates the knowledge of medication administration and evaluates their responses to the medications. Plans for residents’ admission/discharge and anticipates for long term and/or short term residents’ needs.
The LPN is responsible for providing care to a group of patients/residents under supervision of an RN. Effectively interacts with patients/residents, families, responsible parties, and other health team members to assure resident safety. Performs initial data collection on all residents and participates in implementing the care plans based on the residents’ needs. Demonstrates the knowledge of medication administration and evaluates their responses to the medications. Plans for resident discharges and collaborates with the Registered Nurse and discharge planner regarding long-term and/or short-term needs.
