Nurse Practitioner, with wound care experience: In office and at-home Wound Care

Job Description:
We are seeking a compassionate and skilled Nurse Practitioner to join our dedicated wound care team, providing exceptional, cutting edge in-office and at-home wound care services to patients in Jacksonville and the surrounding areas. Our ideal candidate is committed to delivering high-quality, personalized care with an exceptional bedside manner. This position offers the unique opportunity to work closely with patients in our office or their homes, ensuring patients receive the best customized care and support, along with a flexible schedule for the NP. Key Responsibilities:
- Provide comprehensive wound care management and treatment in patients' homes or facility.
- Perform thorough assessments and develop individualized care plans tailored to each patient's needs, based in collaboration with the physician’s protocol.
- Monitor and chart patient progress, adjusting care plans as necessary, in conjunction with the medical team.
- Educate patients and their families on wound care techniques, infection prevention, and overall health maintenance.
- Collaborate with a multidisciplinary team, such as cardiologists and infectious disease specialists, to ensure coordinated and effective patient care.
- Maintain accurate and timely patient records using electronic health systems.
- Adhere to all regulatory and practice standards to ensure the highest level of patient safety and care.
- Participate in ongoing education and training to stay current with best practices in wound care.
Qualifications:
- Master’s or Doctoral degree in Nursing.
- Current and unrestricted Nurse Practitioner license in the state of Florida.
- Minimum of 5 years of experience in wound care or a related field.
- Certification in wound care a plus.
- Strong clinical skills with the ability to perform advanced wound care procedures.
- Exceptional bedside manner with a patient-centered approach to care.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Must be highly detailed.
- Valid driver's license and reliable transportation for travel to patient homes.
