The Substance Abuse Case Manager will be responsible for case management of Suboxone patients, work as the liaison with the Suboxone Providers, Integrated Behavioral Health Providers, other MAT team members and other community agencies, as well as provide direct services to patients as appropriate
Qualifications:
- College Degree and Certification in Substance Abuse Counseling preferred
- Knowledge of Recovery Support Groups/Meetings and process
- Experience in SUD, Crisis Management, Health and Human Services, or Case Management, strongly preferred
- Understanding and familiarity with medical terminology preferred
- Bi-lingual preferred
