Job Summary:
As a Registered Nurse (RN) Case Manager, you will play a crucial role in delivering expert nursing care, managing comprehensive patient assessments, and ensuring the highest quality of care. You will be responsible for coordinating all patient care services, overseeing the care provided by our dedicated home health aide staff, and educating patients and caregivers on disease management, medications, and personalized treatment plans.
Responsibilities:
- Provides skilled nursing care including administration of medications and treatments
- as prescribed in the physician’s plan of care for patients in their home setting.
- Collaborate with all disciplines and primary caregivers to coordinate patient care.
- Conduct comprehensive assessments and update care plans, ensuring patient needs are met.
- Participate in weekly Interdisciplinary Team Meetings.
- Maintain and process all documentation, utilizing our EMR system.
- Instruct, orient and supervise other clinical personnel.
- Will perform other duties as assigned.
Qualifications:
- Licensed Registered Nurse in the state of Connecticut with at least 1 year of clinical experience.
- Graduate of an accredited RN program.
- Home Care and Oasis experience preferred.
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care.
- Basic computer skills and excellent documentation capabilities.
