The RN OASIS specialists conducts a comprehensive assessment at required time points according to CMS guidelines. The Registered Nurse OASIS Specialists implements a comprehensive review and interpretation of Outcome Assessment Information Sets, ensures optimal coding and accurate baseline and subsequent information about patient functional and clinical status. The OASIS specialist effectively uses assessment information to improve patient care and outcome improvement. The Case Manager coordinated the overall interdisciplinary plan of care for a patient, from admission to discharge in a home care enviroment. Fullfill the role of liaison between patient/famild, other health care providers, and home care personnel to ensure necessary care is provided promptly and effectively.
