POSITION SUMMARY
Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required. Includes advanced practice nurses such as: nurse practitioners, clinical nurse specialists, certified nurse midwives, and certified registered nurse anesthetists. Advanced practice nursing is practiced by RNs who have specialized formal, post-basic education and who function in highly autonomous and specialized roles.

Essential Functions Statement(s)
- Monitor, record, and report symptoms and changes in patients' conditions.
- Maintain accurate, detailed reports and records.
- Performs patient comprehensive assessments and collaborates with patient multidisciplinary care team (PT, OT, ST, MSW, and paraprofessionals) and other health care professionals to develop/modify and implement an individualized patient plan of care as per physician orders that ensures quality, proper discharge planning and achieves desired outcomes and goals.
- Documentation of patients' medical information and vital signs.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
- Safe reconstitution and administration of medications in the home
- Direct and supervise less skilled nursing or health care personnel.
- Consult and coordinate with health care team members and physicians to assess, plan, implement and evaluate patient care plans.
- Monitor all aspects of patient care, including diet and physical activity.
- Instruct individuals, families and other groups on topics such as health education, disease prevention and childbirth, and develop health improvement programs.
- Assess the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.
- Inform physician of patient's condition.
- Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.
- Perform other duties as assigned by management.

SKILLS & ABILITIES
Education : Graduate of an accredited university in nursing
Experience : Two years related experience as a registered nurse

Certificates & Licenses/Qualifications
- Valid, unencumbered RN license in the State of Louisiana
- Valid driver’s license without restrictions. Must possess a motor vehicle in good working condition,
which is available during work hours, with current proof of insurance
- Current CPR certification with American Heart Association

Pay: $32.00 - $35.00 per hour
