- Here’s What You Need
- Bachelor's Degree, Bachelor of Science in Nursing (BSN) is required. (CLIAFE) Must comply with CLIA regulations regarding foreign education equivalency required.
- 3 years of RN experience in an acute care hospital is required.
What You Will Do
Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner. In addition to the required qualifications, a successful RN will:
- Serves as a point of contact for all identified stakeholders. Attempts to schedule PCP, specialist or clinic follow up appointments for patients.
- Promotes discussion and assists in the identification of a primary care physician (PCP) for patients without a PCP.
- Clinical review staff requirements and communications

- 1 year of discharge planning and/or care management is highly preferred.
- Must hold an active RN license - Registered Nurse license Upon Hire is required.
- CPR – Cardiopulmonary Resuscitation is required upon hire.
- ACM – Accredited Case Manager upon hire is preferred OR CCM – Commission for Case Manager Certification or ACM – American Case Management Certification upon hire is preferred.
