Position Summary

The Human Trafficking Case Manager will provide services to victims of human trafficking in the Central and Eastern Oregon region of the state utilizing CYFC’s in-house services, contracts, partner agencies, and other collaborations. This position will provide outreach, public awareness and training on human trafficking in the service areas. The Case Manager serves as primary counselor and treatment provider for trafficked clients. Ensures daily needs of clients are met and assists them in working towards, understanding and implementing therapeutic goals and objectives as outlined in their individual treatment plan. Responsible for accurate and timely written documentation of services and data collection. This position is also expected to assist in providing services and written documentation as needed to any resident of the LOFT.

This position is a full-time, 40 hours per week position with variable shifts including days, swing, weekends and holidays. This position includes on call responsibilities.

Duties

  • Administer assessments and intakes of clients upon program placement.
  • Directly responsible for case management and safety planning.
  • Directly responsible for assisting victims with access to short and long term housing options.
  • Directly responsible for scheduling, transportation and documentation of clients appointments relative to physical needs: medical, dental, clothing, etc
  • Directly responsible for scheduling, transportation and documentation of clients home visits, authorized leaves, and school and/or vocational meetings
  • Directly responsible for developing and maintaining case files as required by contract and Agency policy
  • Provide individual counseling as needed to develop and meet treatment goals, objectives and contract obligations
  • Support and/or facilitate and document a weekly independent living skills group
  • Document client behavior in daily log and as appropriate in case notes
  • Directly responsible for developing and documenting Service Plans for all clients
  • Contact and liaison with community partners relative to client needs
  • Participation in recreational, daily living and discretionary activities with all clients
  • Regular, documented communication and contact with appropriate community partners
  • Attendance at case management meetings, staff meetings and any meetings pertinent to clients on caseload
  • Assist in development of required quarterly and semi-annual reports as required by contract
  • Participate in professional development opportunities as available and appropriate
  • Participate in Deschutes County CSEC Response Team as necessary
  • Supervision and positive behavior management/mentoring of all clients
  • Manage client wait list including screening applicants and referrals
  • Provide on call support on scheduled days to include contact with hospitals; law enforcement; legal team; trafficking victims and their families.
  • Documentation of daily log and case notes of any clients treatment or behavioral information needed for clear and effective communication for staff shift change
  • Assist other staff members in providing quality program to all clients
  • Maintain confidences regarding client and staff information
  • Provide training and technical assistance to community members and agencies on issues related to human trafficking
  • Participate in outreach activities and opportunities targeting high risk youth/clients and relevant service agencies
  • Perform other duties as requested by Projects Director or Operations Director

Qualifications

  • Knowledge, Skills and Ability: Ability to develop an environment where clients can succeed by ensuring the atmosphere and basic supplies for education, work, activities and similar are provided. Ability to effectively facilitate and document individual counseling and groups. Ability to interface effectively with other staff, referring agencies, parents, schools and others and maintain a professional demeanor at all times. Ability to provide crisis intervention to clients and remain calm and in control during crisis situations. Ability to work independently and cooperatively as a team. Demonstrate strong written, verbal and interpersonal communication skills. Knowledge regarding issues related to runaway, homeless and street youth; domestic violence; and human trafficking.
  • Experience and Training: A master’s degree with major study in social work or a closely allied field and one (1) year of experience in the care and rehabilitation of client, or a bachelor’s degree with major study in social work, psychology, sociology or a closely allied field and a minimum of two (2) years of experience in the care and rehabilitation of clients, or equivalent combination of education and experience. Experience working with victims of Domestic Violence preferred. Bi-lingual (Spanish/English) preferred. Valid Oregon driver license and CPR/1st Aid certification required.
