Schedule is M-F , hours can be flexible. This position does take on-call rotation.
The MDS office currently consist of 1 RN designated as the NCM and 1 LPN AN ( the AN's do report to the NCM in this department). Licensed for 105 beds.
Prefer prior experience in MDS department, but not required! Also prefer RN but will consider LPN applicants.

The Assessment Nurse (AN) assists in planning, implementing, and evaluating nursing care with the Resident Assessment Instrument (RAI) coordinator. If the AN is a Registered Nurse, the AN may be designated as RAI coordinator. The AN assists with the pre-admission screening process, admission process and nursing interventions. The AN is responsible for clinical documentation to include observing, recording, and reporting to the appropriate person the signs and symptoms that may be indicative of a change in the resident. The AN communicates with the physician, nurses, nurse aides, interdisciplinary team members, residents and resident representatives as to resident’s condition or response to treatment. The AN follows and completes the RAI process including documentation requirements, Minimum Data Set (MDS), Care Area Assessment process, care plan development, and care plan implementation. The Assessment Nurse is responsible to the Director of Nursing
