Essential Duties and Responsibilities:
Update Clinical Record on an ongoing basis as relates to communications, orders, re-hospitalizations, conflicts, etc.
Evaluate and address customer concerns, including documentation and resolution of same.
Assists agency personal in care coordination of patient/client services. Serves as a liaison between the organization, referral sources, patient/clients and agency personnel.
Demonstrate best practices and best clinical care for all patients.
Assists with all aspects of Tele-Health including but not limited to device management and Medalogix.
Performs role of liaison between Tele-Health vendor and agency.
Demonstrates ability to serve as a liaison between Physicians, field staff and clinicians, patients, Tele-health Team, and Nursing Supervisors.
Initiates coordination of services and interdisciplinary communication if indicated based on clinical triage; Cardiac experience required.
Responsible for all flags and task screens.
Performs telephone assessments as indicated.
Responsible for any and all follow up relevant to the assessments.
Will request follow up nursing visits as needed based on triage of appropriate clinical information obtained during telephone contact with patients.
Builds rapport with patients through regularly scheduled interaction.
Oversees aspects of the Tele-Health device program including monitoring of data, physician and primary clinician follow up, maintenance, dispatch, and installation/ discharge of patients.
Responsible for reviewing referrals in getting monitor out to patient's residence, scheduling trouble-shoot visits, and subsequent pick-up of equipment s/p patient discharge from program.
Keep abreast of changes in the virtual care industry.
Responsible for working with Tele-health equipment installers for best practices.
Participates in ongoing patient education
Will need to work weekends as needed.
All other duties as assigned by Management
Must be able to use good and sound clinical judgment.
Must be able to document accurately and appropriately.
Must work well in a team environment.
Maintain confidential information.
Must hold, in good standing, a license as a Practical Nurse issued by the in which you work
Must be empathetic, non-judgmental, tactful, responsible, professional and organized.
Proficient in Microsoft Office Suite.
Proficient in HCHB, HBSS and Outlook.
Education and/ or Experience:
A minimum of two (2) years of experience working as a LPN including some medical/surgical experience and/or an acute care or home care setting is preferred.
Minimum of one (1) year of management experience preferred.
Medalogix experience preferred.
