This position is responsible for providing nursing care, interventions, education,and support to patients and families every other weekend from 8:00am - 8:30pm, Saturday and Sunday. Eligible for a Weekend Shift Differential.
RESPONSIBILITIES:
- Assesses patient’s condition, documents appropriately and updates Care Plans as needed.
- Educates Patient/Family/Caregiver/Nursing Home Staff on administration of medication, symptom management and pain control as directed by physicians and Northern Illinois Hospice protocol when applicable.
- Records observation of reactions and any side effects of medications.
- Informs the Team Leader and Case Manager of the patient's disease progression and all other appropriate clinical matters concerning the patient/family unit for which she/he has responsibility.
- Maintains current knowledge of Medicare/Medicaid benefits, private insurance, supportive care, continuous care and in-patient requirements.
- Educates patient/caregiver on proper and safe use of durable medical equipment and oxygen administration.
- Uses good body mechanics when lifting or transferring patients, utilizing equipment when necessary.
- Maintain Standard Precautions and uses necessary personal protective equipment, in order to maintain a safe environment in conjunction with OSHA regulations.
- Requests counseling/volunteer services as indicated by patient and/or family to provide spiritual support, emotional support and respite services to family and caregiver.
- Completes documentation at time of visit,including but not limited to, Visit Reports, Care Plans, IDT Reports; Trave and Mileage Logs.
- Serves as the patient and family advocate by communicating with the attending physicians, long-term care facilities staff, case managers and others as necessary.
- Coordinates all service delivery after hours including patient and family education and coordination of services with external lenders and resources as needed.
- Documents interactions with patients, families, contacts and vendors in the patient record to include the assessment, plan of care, caller agreement to the plan and understanding of any action and interventions. Document the resolution of each intervention and the patient/family interaction.
- Conduct phone triage and,a follow-up visits to ALL phone calls/request UNLESS the facility/family refuse AND the hospice nurse feel there is no need for a visit. This includes nursing homes and private residences.
- Collaborates with primary physician on an ongoing basis to ensure optimal symptom control and continuity of care plan.
EDUCATION and/or EXPERIENCE:
- Minimum Associates Degree in Nursing; Nursing Diploma,BSN preferred.
- Current Professional Registered Nurse license issued by the Illinois Board of Nursing
- One year of Medical/Surgical, Oncology, or Hospice Nursing experience.
- CPR Certification required.
- Must have a valid Driver's License, maintain current auto insurance coverage and have access to a reliable automobile.
