Coordinates all aspects of patient care for the Partial Hospital Program. Coordinates multidisciplinary treatment plan and planning process. Provides educational group experiences for inpatient and PHP patients. Maintains accountability and responsibility for efficient, effective delivery of patient care services through appropriate utilization of hospital resources. Responsible for proactive discharge planning in collaboration with the medical staff and ancillary departments within the integrated delivery network for provision of optimal patient care outcomes within the continuum of care. Responsible for negotiation of cost-effective care among clinical team members, third party payers and external agencies providing extended care. Requires minimal supervision.
Coordinates all elements of patient care for patients admitted to the partial
hospitalization program: admissions, treatment, discharges, and transfers
from inpatient to partial hospitalization.
Plans, communicates and organizes the multidisciplinary treatment team
initial and review of plan of care.
Provides patient specific medical record information to insurance companies
in regards to severity of illness, intensity of service, discharge screens,
concurrent services provided, and both routine and complex discharge
planning issues in order to obtain authorization of services provided
throughout the continuum of care.
Provides initial pre-certification of insurance coverage for Partial
Hospitalization Program patients.
Communicates utilization review information to the patient’s attending
physician and the multidisciplinary treatment team.
Documentation in the medical record concise and relevant to the patient's
program, plan of care, and discharge planning information.
Facilitates quality management activities related to treatment standards
through concurrent audits of the medical record. Collects data and reports
trends or situations of non-compliance of standards to Director of Behavioral
Provides patient educational group experiences for inpatients and Partial
Hospitalization Program patients.
Displays non-judgmental acceptance, maintains confidentiality, and
establishes a positive rapport and working relationship with
Participates in leadership and collaboration with all members of the health
care team for identification of opportunities for process improvement while
maximizing efficient use of resources and minimizing delays in service.
Minimum Position Qualifications:
Education: Licensed Social Worker or Licensed Chemical Dependency Counselor
Experience: One year in social work or chemical dependency; progressive psychiatric experience. See necessary knowledge, skills and abilities above.

License/Certification: Current Texas license as licensed clinical social worker or licensed chemical dependency counselor.
Preferred Position Qualifications:
Minimum of two years psychiatric experience
Minimum of two years of providing psycho-educational group experiences for mental health patients.
