We are looking to add a Clinical Manager to our management team to support the Director of Clinical Services, Administrator, and Case Management team.

This is an office-based job with some field work as needed and directed by the Director of Clinical Services.

Clinical Manager – Home Health Care Division Job Duties and Responsibilities:

1. The primary result achieved by the Clinical Manager is compliance with clinical documentation, compliance with following physician ordered plan of care, and to provide support and guidance to clinical and office staff as warranted

2. Scheduling for Skilled Nursing Division professional staff weekly and as needed. Ensuring that visits are completed on time and physician orders are followed

3. Ensure compliance with insurance authorizations, including submission of initial and ongoing clinical documentation requested for authorization to be issued for services in specific authorization time frames

4. Oversight of therapy service delivery and documentation

5. Obtain orders for skilled and non-skilled service as assigned by Director of Clinical Services by preparing and submitting needed documentation

6. Collaborate with schedulers for Home Health Aide services

7. Supervision of field nursing staff as it relates to day to day operations

8. Perform in-office case management by communicating with patients, case managers, other community members and all referral sources to facilitate appropriate services

9. In conjunction with Director of Clinical Services, performs document review and completion in computer system, and signs LPN notes and orders

10. Collaborates with Director of Clinical Services about topics for monthly staff meetings and attends all staff meetings. Coordinates interdisciplinary team meetings as directed by Director of Clinical Services

11. Performs nursing functions such as SOC assessments and skilled visits in times of staff shortage under direction of Director of Clinical Services

12. Participates in on-call rotation as determined by Director of Clinical Services

13. Completes initial and yearly competency testing for all staff/LPN/RN. Directs RN's to perform yearly competency testing for paraprofessional staff

14. Investigates and reports any problem relating to client care or conditions which might harm the client and/or employee well-being and immediately reports all incidents to the Director of Clinical Services or Administrator

15. Monitors equipment and supplies for appropriate use and takes steps to keep misuse to a minimum. Ensures that all necessary supplies and equipment are available

16. Ensures compliance with Federal, State, CHAP, local and Agency policies in all client care aspects of the Home Health Care Division

17. Acts as Director of Clinical services in the Director of Clinical Services absence. Provides leadership and guidance to other nurses

18. Addresses any personnel issues to promote a productive and supportive work environment

Experience Needed for Clinical Manager – Home Health Care Division:

1. Minimum of five years of nursing experience with two (2) years in Home Health nursing

2. Must have prior supervisory and management experience in Home Health
