The Licensed Practical Nurse shall interact with the residents during the shift and assist staff in meeting the functional needs of the resident. The LPN shall always assist to ensure a safe and therapeutic environment. The LPN shall work alongside the Registered Nurse to carry out physician orders, prepare, and administer resident medications, assess, and document resident’s condition, and facilitate outside diagnostic or medical services that are ordered by the physician
Position Responsibilities
Essential Responsibilities

Provides assistance to Registered Nurse in identifying resident needs, problems, capabilities, and limitations.

Communicates with other clinical team members observations and recommendations for residents’ plan of care, providing input into the treatment team process.

Documents the administration of medication in according to established policies and procedures.

Coordinates all pharmacy and laboratory orders, delivery, and refills with the appropriate vendors.

Responsible for the security of all controlled medications kept in the facility and monitors the administration and accurate recording of use.

Administers medications according to physician orders and medication administration record.

Able to administer several differing types of injections including Tuber sol PPD reading, Long-Acting Antipsychotic injectables, other Intramuscular Injections as ordered.

Documentation reflects ongoing assessment of resident behaviors and feelings as expressed and are addressed appropriately within the program or with referral.

Provides education to residents and families about medications, side effects, contraindications, and physical symptoms through individual counseling and group counseling.

Provides review of clinical records for accuracy and quality documentation.

Assists in the coordination of admissions, transfers, and discharges to provide a smooth and efficient flow of communication.

Coordinates facility infection control program: ensuring staff and residents meet policy and procedure requirements.

Minimum Requirements
Education and Experience

Graduate of an accredited school of professional nursing.

Requires state license as a Licensed Practical Nurse.

Prefer one (1) year experience as a Licensed Practical Nurse. Prefer at least one (1) year in psychiatric nursing. Knowledge of current nursing procedures, the nursing process, medications, and their correct administration. Must have knowledge of human development over the life span.

Requires valid CPR/BLS and State Driver’s License.
