Great Care Home Health is in search of a LSW, LPN or RN to provide Health Care Coordination to patients in their home. This role will primarily work providing support to patients by collaborating with physicians, identifying unmet needs, making appropriate referrals, medication review, advanced care plan development for in-home services and oversight of a healthcare support plan that proactively manages both chronic and complex medical conditions, support during transitional care from hospital or nursing facility to home/assisted living.
The ideal candidate will have home health or long-term care experience, promote and advocate patient’s rights, proactively and creatively problem solve patient issues impeding patient well-being, be (or become familiar) with other home and community-based services available to the patient. And, most importantly, a candidate who is passionate about improving the health and well-being of every patient.
- Part Time to Full Time if you choose to increase patient case load.
- Shifts Monday – Friday 8:30a.m. to 4:30p. Work outside of those hours would be only a necessity to meet patient unique circumstances.
- Rarely weekend or holiday work hours.
- Set your own schedule.
- Minimum of monthly in-home patient consultation.
Do you have what it takes?
· Valid Indiana RN or LPN license or LSW with Masters degree in social work.
· Valid Indiana Driver’s License, reliable car & auto insurance
· CPR and Basic Life Support certification
· Clean criminal history
· 3+ years of experience preferred.
· Management experience required.
· Ability to professionally write and effectively communicate.
· LSW work experience: Two years of experience providing health care coordination.
