Bachelors degree required. Formal training in project management such as that required to achieve certification in project management is preferred.
Must have a minimum of 5 years of management level experience in a healthcare organization. Must possess a strong knowledge and understanding of physician practice as normally demonstrated through at least five years of experience in healthcare operations business or related field. Demonstrated knowledge of physician practice operations and regulatory compliance in healthcare. Demonstrated working knowledge of comparative data analysis and lean process improvement is preferred. Knowledge and demonstrated competency with of a variety of project management tools required. Working knowledge of Microsoft Office suite of applications Internet and online sourcing also required.

Demonstrates advanced understanding of planning and project management concepts. Able to conceptualize envision and plan for the future. BACKGROUND: The primary care service line has continued in its standard work and would like to use the PCMH Recognition process to perform gap analyses and to evaluate and implement processes and procedures that will improve the care for patients and make our practices more efficient. The standard work that would be created from this process will better allow PPG to meet our quality metrics.
Clinical Operations has been focused on implementing and training on standard work. The Directors of Operations has been focused on supporting their growing territories, providers and managers. There are 40 Core Criteria and 25 Elective Criteria that need to be met to become PCMH Recognized. There is no designated person that can oversee and own the PCMH journey.
RECOMMENDATION: PCMH is a large project that we currently do not have designated resources for. Utilizing a PPG PCMH RN Manager allows for a dedicated resource to spend time in the pilot sites to evaluate, implement and execute our processes to become PCMH designated. The role would be responsible for working collaboratively with leadership, providers, and co-workers to provide excellent service to our patients. In conjunction with operational and physician leadership, this individual would serve as a leader and champion to set the tone for how the practices will function as a medical home.
Job Responsibilities Would Include:
A. Assembling/monitoring program structure
B. Documenting guidelines and protocols
C. Work with Clinical Operations on Gap Analysis
D. Developing processes and workflows to meet Core and Elective Criteria
E. Care Gap Closure initiatives
F. Documenting Quality Improvement activities
G. Collaborating with departments to confirm gaps in preventative care
H. Work with IT/EPIC on coordinating changes in the EHR and reporting capabilities
I. Analyzing data
J. Performing reporting functions
K. Preparing application documents
L. Training staff on PCMH functions
