Schedule: Mon-Fri 7:30am-4:00pm plus weekend and on-call rotation

Summary of Job Duties:
Under the supervision of the Home Care Supervisor and working closely with the Interdisciplinary Team (IDT), the Home Care Coordinator is a COTA/L provides the technical, organizational and communication skills necessary to efficiently assess the need for, and coordinate the delivery of, home / personal care services for adults age 55+. The Home Care Coordinator must demonstrate the knowledge and skills necessary to plan care for and provide service to frail elders according to assigned responsibilities, standards of practice in gerontological nursing and Community LIFE standards. Mileage is reimbursed for home visits.
Required Education: Graduate of accredited Certified Occupational Therapist Assistant (COTA) program.
Required Experience: At least one year of experience working with a frail elderly population, preferably in long term care, home care or community health. Experience teaching patients and other health care workers preferred.
Required Certifications/Licensure: Valid PA COTA license and Valid PA Driver’s license.

Working knowledge of local health care and geriatric service networks.

Basic working knowledge of Windows operating systems, e-mail, word processing.

Able to deliver services in a compassionate, responsive, and courteous manner. Dependable, resourceful and flexible.

Able to work effectively with staff, participants, providers and referral sources.

Interest in geriatrics and community-based programming.

Ability to appreciate and enjoy working with elderly individuals.
