Patient Care Coordinator
LeMed Specialty Pharmacy is a URAC and ACHC accredited community specialty pharmacy located in the Bronx, NY. We work on a close basis with patients and providers to improve health outcomes through careful coordination, patient education, and a focus on removing all barriers to patients' receiving the treatment that they need.
The Patient Care Coordinator is a key member of our team, and central to our ability to effectively serve our patients.
We are seeking candidates with a background in pharmacy, ideally in a patient or provider facing role. The ideal candidate has experience in working with patients as a patient advocate/pharmacy technician--focused on helping patients understand their treatment plans, providing refill reminders and regular communications and inquiries about patient wellness, coordination with patient heath care teams, and also providing patients access to resources and educational programs to help them better manage their therapy.
Comfort with and a passion for working with providers, patients and growing our specialty pharmacy business is a must. The successful candidate should possess strong organizational, interpersonal, and time management skills.
Duties and Responsibilities include the following:
- Acts as liaison between company, patients, and providers' offices.
- Explains all company programs and services to patients and prescribers' staff.
- Communicates with patients as needed based on prescription refills and refers any medication or illness related questions or concerns to a board licensed healthcare professional including but not limited to: nurses, pharmacists, or supervised pharmacy interns.
- Responsible for processing prescriptions and refills through pharmacy dispensing software.
- Responsible for documenting patient and prescriber interactions using therapy management software
- Contacts patients to schedule deliveries and notify of payment responsibilities.
- Keeps current with new treatment trends.
- Communicates patient statuses to prescriber offices and clinics
- Maintains patient confidentiality
- Performs other duties and responsibilities as assigned.
Qualifications:
Required Education and Experience:
- High school graduate or equivalent
- Three years experience in a pharmacy setting or one year experience in a URAC or ACHC accredited pharmacy (preferred)
Required Professional Competencies:
- Basic computer skills are required
- Able to work independently, as well as part of a team
- Strong written and oral communication skills
- Must be accurate, detail oriented, reliable and able to manage multiple tasks
- Organizational skills to maintain records/information
- Speaks Spanish (preferred)
Licenses/Certifications: Active Certified Pharmacy Technician certification (CPhT) (preferred)

- This position works 40 hours per week on weekdays and on Saturdays. The weekday shift is 10-6:30pm and the Saturday shift is 9-4:00pm. Are you available to work during these shifts?
