Are you looking to take your career to the next level? We are looking to fill a **Community Impact Director** position with a person experienced in helping diverse communities in Connecticut achieve health equity.
**Your Key Responsibilities will include: **
**Health Lead to the Community: **
+ serve as the “health lead” for the region by engaging and mobilizing the community;
+ managing a collective impact plan;
+ supporting heart healthy resources in the community;
+ building strong volunteer relationships.
**Building the Collective Impact Plan** : Together with community residents and partners:
+ continually assessing the health needs of the community;
+ developing, coordinating and/or implementing community wide strategies;
+ leading a prioritization and action planning process that focuses on eliminating health disparities.
**Support Blood Pressure & Cholesterol Improvement: **
+ work with healthcare providers in the local market to ensure standard methodologies for treatment of patients through initiatives and programs;
+ consulting with clinical staff for data entry, data reporting, compliance issues, and strategies for improved patient outcomes.
**Engage & Empower Volunteers: **
+ accountable for the recruitment and management of volunteer networks;
+ building a network of meaningful volunteer partnerships to advance the mission;
+ providing timely direction, important opportunities, resources and recognition to volunteers while at the same time relying on their expertise, abilities and willingness to use their networks to drive the goals of the organization;
+ holding both volunteers and staff accountable, while being accessible and building collaborative staff-volunteer partnerships that are based on a foundation of mutual trust.
+ drive collaboration with internal staff partners in the market around mission related goals;
+ work together to ensure health strategies are coordinated into development, communications and marketing, advocacy, and healthcare quality improvement activities;
+ collaborate with staff partners and key volunteers to identify, cultivate and secure program funding, including sponsorship, for relevant mission campaigns.
In this role, you will report to the VP Community Impact and will be based in our Wallingford, CT office.

+ having a bachelor’s degree from an accredited university is preferred;
+ having at least 5 years’ experience working with and/or handling community/public health promotion or related educational programs is needed;
+ you are a skilled organizer with experience in community engagement and coalition management including multicultural communities;
+ you have the ability to interact across multiple acculturation levels and socio-economic groups;
+ you are familiar with the geography, key institutions and key partners and volunteers;
+ experience convening and leading partners, building strategic partnerships, and conducting meetings with internal and external clients including interacting with external partners/sponsors as necessary;
+ critical thinking skills in assessing the local market environment and available resources to successfully implement new and existing programs;
+ able to lead projects and events ensuring deadline compliance;
+ you can efficiently participate in a multi-disciplinary team environment working towards common goals;
+ you can influence team members without supervisory authority;
+ have highly effective organization and analytical skills, multi-tasking, communication, negotiation, interpersonal skills and are proactive;
+ ability to recruit, mobilize and lead volunteers.
+ intermediate to advanced skill with Microsoft Office 2010 or higher used for word processing, email, presentations, and spreadsheets.
+ Ability to travel around the state and transport items to and from meetings and events.
+ willingness to occasionally travel outside the market and to work evenings and weekends as needed.
