Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) provides direct services to individuals who are homeless. Services include outpatient primary health care, behavioral health care (individual and group mental health and addiction treatment), and outreach and case management services. Working together, our integrated, multi-disciplinary treatment teams address physical and mental health needs, substance use disorders, and identified social determinants of health by providing access to a wide-variety of patient-centered, trauma-informed treatment and services. CHL HOAP is a Federally Qualified Health Center (FQHC) Health Care for the Homeless (HCH) program.
The live-in house manager position provides monitoring and assistance to clients of the HOAP Transitional Housing Program. They will monitor transitional house and premises in the absence of Housing Coordinator.
Works closely with Program Coordinator to monitor premises and clients in Transitional Housing.
Monitors the clients and premises during evening and weekend hours when Coordinator is not on duty.
Maintains an on-going log of significant events and review this log with Coordinator on a routine basis.
Identifies crisis problems and alerts public safety professionals (POLICE, FIRE DEPARTMENT, AMBULANCE) as appropriate.
Notifies Coordinator of crisis problems or potential safety concerns immediately.
Agree to respect the privacy of the residents of this program and keep confidential their identity, living address and any information you may acquire concerning their Mental or Medical health and any related conditions.
Promptly inform Program Coordinator regarding any maintenance issues at the property in a timely matter.
Performs nightly rounds in all apartments to ensure clients are in by curfew.
Set up a meet to great appointment with all new clients.
Distribute toiletries to all apartments weekly.
Maintain a professional and respectful working relationship with Coordinator all clients at all times.
Attend monthly supervision with Program Coordinator.
Attend all mandatory house meetings.

Minimum of HSD, HiSet or GED required
Previous experience working with substance abuse population
Ability to maintain the confidentiality of clients
Ability to work and communicate in a professional manner
Must be able to pass a CORI background check
