Assesses patients as assigned using both objective and subjective data
Develops a plan of care in collaboration with the multidisciplinary team and mutually setting achievable goals with the patient/family/significant other
Implements the plan of care considering the patients age-specific and individualized needs
Evaluates patient outcomes and documents in accordance with Center policy
Serves as the patient advocate
Organizes workload appropriately and completes patient care assignments equitably and efficiently delegating tasks to other care team members in accordance with their job descriptions and capabilities
Ensures appropriate completion of patient documentation, incident reports, computer data entry, and all other Center forms per policy
Relays patient information to other care providers notifying the Director of Nursing of potential problems/hazards
Provide input and carry out Center policies and procedures
Works to the extent permitted by licensure under Pennsylvania Board of Nursing Practice Standards
Is able to plan and organize their daily assignment and seek out assistance and guidance as necessary
Follows cost saving measures as directed by the Director of Nursing and Administrative Director/Director of Nursing
Participates on committees as assigned
Assists in Quality Improvement activities to ensure high quality of patient care
Attends at least one In-service per year
Other duties as assigned
