Full-Time: Monday 8 AM to Friday 5:00 PM Hours may vary at times to meet the needs of clients.

The Population Health Administrator supports the development and implementation of client-centered and interdisciplinary team-based care. This position will support all departments and programming including: primary care, psychiatry, pharmacy, care coordination, peer support, therapists, nurses, and quality management. The Population Health Administrator will assist in the management of panels across teams by gathering, organizing, and inputting patient data within the electronic health record and Health Current/Health Information Exchange. The Population Health Administrator will serve as a representative for payors and insurers. The Population Health Administrator will help to identify client needs, gaps in care, and engagement opportunities for staff, stakeholders, payors, and clients. This position will gather summary information for evidence-based treatment interventions, self-reported outcome measures, and screening tools. Ongoing communication with the interdisciplinary team members and training are vital parts of this role. The goal of this position is to facilitate high-value, client-centered care that improves access to care, client outcomes, quality improvements, fidelity monitoring, and focuses on preventative services and chronic disease management. The Population Health Administrator will assist in process mapping and developing workflows for population health management within the various programs of Escalera. The Population Health Administrator will ensure best practices, high-quality standards, pursuing the goals of the Quadruple Aim, participate in developing new tools, reports, dashboards, and program development.
Required: Bachelor’s Degree, 3+ years of experience in healthcare field, previous population health experience, ability run, pull, track, and monitor reports to implement workflows and enhance outcomes
Masters Degree highly preferred
Bilingual preferred but not a requirement
Knowledge of Microsoft Office, PowerPoint, and especially Microsoft Excel
Experience with electronic health records
Strong organizational skills
Knowledge of integrated healthcare delivery model
Proficient in data management and reporting
Proven problem-solver with ability to multi-task
Ability to work with large data sets with guidance from medically trained individuals
Ability to identify problems, think logically – or creatively, and devise practical solutions
Excellent writing and oral presentation skills
