POSITION SUMMARY

Advanced Practice Nurse working with complex institutionalized patient populations and their families on medical issues, to impact their quality of life and reduce preventable hospitalizations. Advanced Practice Nurse Practitioner improve face-to-face care coordination between health care providers and the member (and family or representative) to offer an additional layer of monitoring and support to nursing facility care. The Nurse Practitioner participates in member care conferences, routine psychotropic drug reduction and facility quality improvement and performance improvement meetings. SNFist Advanced Practice Nurse Practitioner continuously evaluates individuals who are strong candidates to transition back in to the community. Discharge planning goals are identified early in the process, and updated based on member’s progress.

Essential duties and responsibilities include the following:

  • Perform initial patient screening, patient history and physical examinations.
  • Assess high impact residents who reside in skilled nursing facilities to halt progression of chronic diseases, intervene in acute exacerbations of illness, and prevent avoidable hospitalizations.
  • Improve quality of members health status and promote cost effective interventions through early identification of health issues.
  • Gives medical recommendations to Primary PCP/NP
  • Analyzes test data and determine the further interventions and further treatment options.
  • Deliver quality care while maintaining Florida Community Care (FCC) metrics pertaining to productivity.
  • Serve as the resource for patient’s needs, family counseling and provide guidance and education for staff.
  • Serves as a consultant and liaison to nursing and other health care professionals in solving complex patient care or system problems.
  • Collaborates with FCC Care Managers to provide stellar care and services.
  • Reviews medication lists and reconcile medication orders to promote wellness and prevent errors.
  • Promotes a high level of member and family satisfaction with healthcare services.
  • Collaborates with FCC Care Transitions team in identifying members for transitioning to community setting.

General Job Requirements

All employees shall meet specified training program including Compliance/Privacy Regulations, and attend at a minimum one (1) hour of Compliance/Privacy educational training annually, as required by Florida Community Care.

All employees shall meet Risk Management Regulations, and attend at the minimum one (1) hour of Risk Management education and training within the first thirty (30) days of employment and as required by law or Florida Community Care; and, for that non-physician in clinical direct care delivery services, annually thereafter.

All employees if duly licensed in the State of employment, and said license is part of requirement of their position with Florida Community Care, shall maintain in good standing and current state licensure.

All employees are required to maintain confidentiality, protect privacy, comply with PHI regulations, and report violations.

EDUCATION and/or EXPERIENCE:

Minimum Education:

  • Master’s degree in Science with certification as an Advanced Practice Nurse.
  • 3 years of RN experience

Other:

  • Maintain CPR certification
  • Maintains certification with a Florida Credentialing Center.
  • Master’s degree in Science with certification as an Advanced Practice Nurse.
  • 3 years of RN experience
  • Maintain CPR certification
  • Successful completion of a Level II criminal history and/or background investigation
