Registered Nurse - Weekends, 4pm to 9pm or 1pm to 9pm
$3,000 sign-on bonus
Requirements:
- Licensed RN in Massachusetts
- Graduate of an approved school for professional nursing,
- A minimum of one year of recent experience in direct patient care nursing.
- Previous home care experience preferred.
- Experience with documentation in a patient Electronic Medical Record is a plus.
Responsibilities:
- Assumes on-call coverage during evening hours and responds to patient/family needs in accordance with CNHC policies and procedures.
- Able to assess and respond to the needs of patients and families in varied settings.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
- Able to make meaningful observations and write comprehensive accurate reports.
- Communicates with attending physicians, other staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.
- Maintain regular communication with the Clinical Manager, and/or other care team members concerning patient/family care needs.
- Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
- Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, or by providing direct care as appropriate.
- Provide appropriate support at time of death and period of bereavement.
- Participate in CNHC’s orientation and in-service training programs for professional staff.
- Participate in agency and community programs as requested to promote professional growth and understanding of home care.
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
- Accept other assignments as appropriate.
