We at Cherishaven Hospice is a Hospice Agency based in the City of Alameda California. We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide end-of-life care and support to patients and their families during challenging times.
Responsibilities:
- Provide compassionate care to terminally ill patients in hospice settings
- Administer medications and treatments as prescribed by physicians
- Monitor patient conditions and report changes to the healthcare team
- Assist with pain management and symptom control
- Collaborate with interdisciplinary teams to develop individualized care plans
- Offer emotional support and guidance to patients and their families
- Ensure patients' comfort and dignity throughout their end-of-life journey
Skills:
The ideal candidate should possess the following skills:
· Minimum experience in Hospice Care
· Experience in Home Health, primary care or critical care settings
· Proficiency in medication administration and case management
· Knowledge of physiology, anatomy
· Experience with admission and care plans
· Ability to multi-task, good time management
· Knowledge of wound assessment and treatment
· Knowledge of EMAR, Hospice MD program
· Have a reliable vehicle for transportation.
