SUMMARY

An employee in this position provides individual and group activities targeted toward residents with Alzheimer’s and/or Dementia. He/she regularly interviews and/or assesses residents to determine their recreational, social and therapeutic needs. The Dementia Activities Coordinator is responsible for organizing and implementing programs and events beneficial to those suffering with dementia related illnesses.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Create, design, and implement activities and events related to persons with a dementia related illness.

Responsible for timely completion of associated charting and documentation related to the provision of activities to associated groups.

Set areas as needed for the type of activity planned.

Remind nursing staff of activities scheduled and residents who desire to participate.

Assist with transport of residents to the activity area.

Follow the instructions for the activity as directed and observe individual resident participation and responses during the activity.

Assist residents back to living areas at the conclusion of the activity.

Notify nursing and complete documentation of resident behaviors which may indicate medical problems.

Interact appropriately and effectively in one to one activities according to the resident's strengths and limitations.

Be aware of individual cognitive, emotional, and psychosocial impairments. Keep records of sensory stimulation or other type of activity and resident's response.

Chaperone scheduled activities away from the facility grounds.

Responsible to maintain a pleasant, upbeat, engaging, and cheerful nursing home environment.

Assist with putting up, taking down, and storing seasonal decorations.

Assist residents to display calendars, pictures, cards, or other mementos in their rooms.

Read mail to residents as needed.

Assist with creating, copying and delivering monthly newsletter and monthly calendars.

Keep desk area, activities office and activity closets organized.

Participate in and promote staff participation in facility-wide special activities such as dress-up days and special observances.

Instructs residents in relaxation techniques such as deep breathing, concentration, and other activities, to reduce stress, aggression, and tension.

Organizes and coordinates special outings and accompanies residents on outings such as ball games, sightseeing, or picnics to make residents aware of available recreational resources.

Prepares and submits requisition for needed supplies.

Knowledge of Federal, State and local regulations relating to recreational services within a long term care facility.

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Ability to understand and carry out oral and written instructions.

Ability to observe and timely and accurately describe changes in behavior and affect.

Ability to establish effective work relationships with superiors, peers and other facility staff; local and regional providers, residents’ families, friends, etc.

Possess the personality and ability to congenially work among residents, peers, families, etc.

Requires strong ability to comprehend, interpret, and create written documentation

Must possess a strong ability to communicate professionally and effectively with residents, families, superiors, and team members
CERTIFICATES, LICENSES, REGISTRATIONS, ETC.

A valid PA Driver’s License is required.
Current and valid Nurse Aide Certificate or willingness and ability to obtain Nurse Aide Certification

EDUCATION and / or EXPERIENCE

Graduation from an accredited high school or an accredited equivalency program.
Two years of experience working with seniors in a skilled care facility.
Minimum of two years of experience with performing and/or developing dementia related activities

Must be able to push/transport residents of various heights and weights in wheelchairs. Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus.

While performing the duties of this job, the employee is frequently exposed to outside weather conditions. The employee is occasionally exposed to wet and / or humid conditions and fumes or airborne particles. The noise level in the work environment is usually loud.
