Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.
Responsibilities:
The Service Coordinator is responsible for the professional assessment of the strengths, wants, and needs of persons with chronic and persistent mental illness, the development of individualized service plans with these identified individuals, and assistance with achieving these service plans goals within the framework of the Service Coordination rules, regulations, best practices, and SPA principles.

Utilizes Sanctuary Models shared values, the Seven Commitments and tools to conduct daily tasks.

Assesses individuals strengths, wants and needs through a series of face-to-face, phone and collateral contacts using the principles of MH recovery.

Assists individuals in the development of individualized, strengths-based service plans after professional assessment has been completed.

Uses knowledge of Service Coordination rules, regulations, best practices and SPA principles to assist individuals with the attainment of service plan goals.

Uses professional judgment to determine the appropriate level of assistance necessary from the SC program to help individuals achieve their goals.

Accurately and efficiently documents all work performed, including, but not limited to, progress notes, individualized service plans, strengths assessments, environmental matrixes, and other collateral documentation as necessary.

Abides by all state regulations, CCBHO Performance Standards, and all SPA principles.

Participates, as scheduled, in emergency on-call rotation which services all SC consumers.

Meets all paperwork/computer work deadlines, and meets or exceeds productivity requirements

Qualifications:
- A bachelor’s degree in the human services field OR an RN certificate OR High school diploma AND 12 semester credit hours in the human service field and 2 years direct client contact

Valid PA driver’s license and reliable transportation

Act 33, 34 and FBI clearances
Minimum Compensation: USD $40,000.00/Yr. Maximum Compensation: USD $42,000.00/Yr.
