Daily and consistently implements approved and scheduled active treatment programs for residents to include, but not limited to: areas of, self-care, daily living, socialization, recreation, leisure, speech, community awareness, and vocational/prevocational skills, ensuring the residents safety and privacy. Provides supervision and training at worksites, working with vocational staff to maximize training and attending to residents’ self-care needs as appropriate. Adheres closely to the residents’ daily activity schedule. Reports progress or lack thereof of the residents’ goals and objectives to supervisor and suggests improvements as appropriate.
Follows approved individualized behavior support plans utilizing approved strategies. Follows occupational therapy, physical therapy and speech therapy programs as prescribed.
Implements appropriate recreation and leisure activities for residents at home, as well as away from home through center wide activities, community outings and attending alternative leisure activity sites.
Accurately documents resident information including, but not limited to: behavioral issues, toileting, food/fluid intake, and active treatment following each activity as necessary. May provide valuable input concerning the observation of the resident and makes suggestions for improvements in the activities under discussion.
Transports and/or escorts residents to and from activities, community trips and appointments, including emergency visits to the hospital to ensure their safety and well-being. Provides care for residents during overnight stays in hospital (until admitted), as needed, in accordance with the established treatment program.
Performs mealtime procedures strictly following each resident’s prescribed diet and assuring appropriate use of prescribed adaptive equipment by assisting the residents as needed. Assures that residents do not eat too fast, food is cut to appropriate size and residents are properly positioned at all times. Ensures that foods are maintained at appropriate temperatures, dates and labels are checked, and that food is properly stored and maintained to avoid spoilage.
Checks residents’ clothing and personal possessions daily, reporting any needs, concerns, and/or repairs to supervisor in a timely manner. Ensures daily that the residents’ personal appearance is neat, clean and their clothes are well-fitted. Completes resident’s Personal Possession Inventory accurately and on a monthly basis as assigned.
Attends to residents’ injuries and illnesses by utilizing appropriate first aid procedures and notifying the nurse/or supervisor as the situation warrants. Accurately completes Incident Reports, and documents illnesses and injuries in the Shift Communication Book within one (1) hour of occurrence.
Provides and receives communication with staff on adjoining shifts regarding significant job-related information on a daily basis and documents such in the Shift Communication Book. Remains on duty at the end of the shift until proper staffing is assured for the oncoming shift. Stays to work the next shift to ensure adequate staffing to meet the needs of the residents.
Attends in-service training and other staff meetings as necessary. Attends mandatory training and/or maintains certification in Prevention and Safety Strategies, Cardiopulmonary Resuscitation and Direct Care In-Service. Completes annual health screening.
KNOWLEDGE, SKILLS AND ABILITIES
Must be able to lift up to 40 pounds.
Must be able to understand and apply the methods and practices used in medical, psychiatric, nursing or child care or in working with developmentally disabled persons.
Must be able to read.
Must be able to pay attention to details.
Must be able to follow verbal and written instructions.
Must be able to carry out instructions for implementing prescribed treatment plans including behavior modification activities.
Must be able to instruct others in basic self-care skills.
Must be able to instruct others in recreational activities.
Must be able to provide instruction and personal guidance and care to residents.
Must be able to communicate effectively verbally and in writing.
Must have knowledge of the methods of instructing and providing care to others.
A valid Florida Driver’s License. (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver’s license within 30-days of hire. License suspended or revoked for any reason, work permits (Business purpose/Employment/Education only license) and some types of restricted license are not acceptable. License that have “Corrective Lenses Restricted,” are acceptable, provided the driver wears the corrective lenses while operating the vehicle.
A high school diploma, and minimum of two (2) years’ experience providing care and treatment for people with developmental disabilities.
This position is located at a 24- hour facility. Staff members must be agreeable to working a shift schedule; regular, evening, night.
Employees may be required to work a double shift, or additional days/hours based upon the needs of the home and center. This includes, but is not limited to: ensuring home coverage, activities, holidays, and emergency situations.
