The clinic coordinator provides essential services that focus on:
1) Each individual patient and family as they move through the clinic.
2) Overall flow of the clinic operations.
The clinic is interdisciplinary, with multiple providers seeing each patient during assessment, treatment and monitoring. The SAC (Swank Autism Center) Clinic Coordinator will coordinate patient visits from first contact with the Swank Autism Center through the patient’s transition to adult care. The SAC Clinic Coordinator will coordinate visits with team clinicians; facilitate referrals to and communication with Nemours’ and community providers; organize and maintain a clinic roster and patient status; and coordinate patient care conferences. The clinic coordinator will frequently act as the patients’ contact point into communication and services by the Center.
Performs appropriate assessment of patient/family upon referral and determines time and provider for initial appointment.
Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care related to patient needs.
Identifies patient/family need for further resources and initiates appropriate teaching and/or referrals.
Participates in responding to patient phone calls. Explains treatment regimens to patient/family after having discussed with the physician.
Demonstrates ability to share clinical knowledge with other members of the health care team; works collaboratively with Nursing Education Department to share sub-specialty knowledge with nursing staff.
Acts as a liaison between families and members of the health care team.
Utilizes specialized body of knowledge and technical skills in nursing practice related to sub-specialty in accordance with the Nurse Practice Act, hospital policy and protocol.
Participates in the development of standards of care for the specialty patient population.
Demonstrates proficiency in the care of all ages; neonates, pediatrics, adolescents and adults.
Recognizes and identifies own learning needs and responsibility for continual development; strives to increase knowledge and identifies goals for professional growth.
Responsible for participation in departmental and hospital programs for Quality/Process Assessment and Improvement, identifying opportunities to improve services, making recommendations and implementing actions as appropriate and consistent with the goals of the Nemours Foundation.
Coordinates care for SAC patients including obtaining intake forms, past medical records, ROI for school records, scheduling diagnostic process/assessment, scheduling SAC appointments, coordinating care with Social Work, coordinating care with SAC clinicians, and scheduling patient care conferences.
Facilitates open communication with Nurse Managers, Clinical Educators, staff nurses, and medical staff, Social work. Maintains a current knowledge base through participation in appropriate professional activities and organizations. Participates on intra- or interdepartmental committee(s) as appointed by Nursing Leadership. Stays current on technological aspects of the position. Ensures written communications are focused to the reader’s understanding

Bachelors degree required.
DE RN licensure required
Three years of related clinical experience preferred
Active Delaware (or compact state) Registered Nurse Licensure Required
