Duties include:
  • Processing the visit schedule
  • Maintaining communication with patient, caregiver and discharge planner if patient is hospitalized
  • Monitoring the status of daily reports
  • Receiving referrals from physicians, discharge planners, etc.
  • Receiving orders from physicians
This job requires a person who can work independently, while never forgetting that home health care is a team effort.

Other requirements include:
  • High school diploma or equivalent.
  • Experience in the operation of automated information systems, personal comuters and PC based applications experience.
  • Must possess knowledge of Clinical Records Systems.
  • Ability to communicate effectively, both verbally and in writing
  • Respecting the patient’s privacy and maintaining confidentiality
  • Current Alabama driver’s license and automobile liability insurance
