We are seeking a Part-Time Family Medicine or Internal Medicine Physician to work in a 100% outpatient clinic. In this position, the Physician is a co-clinical lead of the integrated care team that will plan, coordinate, deliver, and manage comprehensive healthcare services for patients. Ensures patient/provider continuity and access to timely and appropriate levels of care.

Duties:
- Provide complete, comprehensive, family-oriented outpatient care to their assigned panel of patients through the life cycle.
- Provide preventive care, manage acute and chronic conditions, develop a care plan, and coordinate care.
- Make appropriate referrals for additional services.

Pay: $150,000.00 - $170,000.00 per year
