Job Summary
We are seeking dedicated RNs and LVNs to join our Home Health team in Home Health Nursing Agency - referral. The ideal candidate will provide compassionate and high-quality care to patients in the comfort of their homes.
Responsibilities
- Conduct patient assessments and develop individualized care plans
- Administer treatments as prescribed
- Monitor patient conditions and report changes to the healthcare team
- Educate patients and their families on managing illnesses and injuries
- Coordinate with physicians and other healthcare professionals to ensure comprehensive care
- Maintain accurate medical documentation
Requirements
- Valid RN or LVN license in the state of CA
- Experience in Home Health
- Proficiency in medical documentation
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
