Growing Home Health agency seeking experienced Home Health RN. Experience with Kinnser/Wellsky EMR is a plus. If you are an RN we have the position for YOU! This will be a Per Diem paid position. Hours could vary.

RN to Performs OASIS assessments and establishes care plans and coordinates the services needed to address complex chronic illness, disability, or developmental disability. Directs the provision of home nursing care, therapy, supportive services, as well as ancillary services and resources, based on our protocol. Provides nursing care through the competent application of the nursing process. Ensures that patients have access to services and opportunities that enhance their development, autonomy, and independence, productivity, well-being, and capacity for social interaction wherever possible.
