We are looking for a full time Case Manager to grow ExactCare Pharmacy’s business in the Harrisburg, PA market by promoting and educating potential patients about our amazing service. This is a high travel role with potential overnights. The Case Manager will conduct in-home assessments and ensure the safety and compliance with each patients’ medication management. The Case Manager will build relationships with patients by conducting in-home assessments, enrolling patients in ExactCare’s medication management program, providing ongoing assistance to help patients get started, and acting as a liaison between the patient and his/her ExactCare pharmacy team. The position is a fully remote, self-directed, telecommuting/field-based position. We will provide you with all the resources needed to successfully execute your responsibilities, including marketing materials, company credit card, laptop with Wi-Fi, smartphone, GPS unit, car allowance, and gas/oil change reimbursement.

Essential Duties & Responsibilities:
Partner with Clinical Liaison Field Manager in assigned territory to educate facilities, institutions, and private patients about ExactCare Pharmacy to grow assigned territory.
Keep lines of communication open between Clinical Liaison Field Manager and New Patient Engagement Team to address any potential issues that may/may not arise.
Manage the patient assessment process to grow business and maintain referral relationships.
Conduct home assessment meetings with patients and/or their families, health professionals to assess and review their situation related to their medication.
Responsible for accurate data entry in compiling all patient medication with the appropriate prescription information to ensure the patient will receive the exact medication at the exact time.
Support new patient referral volume by participating in assessment scheduling activities.
Maintain accurate records and prepare reports for the consistency of information to be shared with the Exact Care Pharmacy team.
Other duties as assigned.

Excellent communication skills; oral, written, facilitation and presentation.
Proficient in MS Office suite.

Requirements:
Valid driver’s license in good standing.
Ability to be on the road daily, traveling throughout Harrisburg, PA.
Bachelor’s degree preferred; Social work or an equivalent field, preferred.
Experience in field social work preferred.
