Community Care Worker I: determines and re-determines need and eligibility for Community Care for the Aged and Disabled (CCAD) services. Provides direct delivery and contract services to long term care services clients in assigned geographic area. Duties include managing a caseload, establishing eligibility for social services completing needs assessment, and making referrals to contract or community agencies. Work includes operating a computer.
Essential Job Functions:
Attends work on a regular predictable schedule in accordance with agency leave policy. Interviews clients or resource persons and conducts home/site visits to assess the need for services. Determines eligibility for services according to program policy and procedures. Develops and implements service plans for clients. Provides ongoing case management to monitor client condition and assess services provided in resolving client problems. Maintains case records to for a written record for each client. Prepares reports and records to provide management information on program operations. Operates a computer to document, update, and inquire into case records. Performs related work as assigned.
Knowledge Skills Abilities:
Knowledge of gerontology, the dynamics of the aging process, and special problems of the aged and disabled. Knowledge of interviewing techniques to obtain personal information and assist in conflict resolution. Experience communicating effectively with all types of people in may different situations. Knowledge of the English language such as grammar and organization of expression for composing correspondence and reporting casework. Ability to communicate orally and in writing. Ability to listen to clients, exploring and reflecting feelings and present relevant alternatives describing feasibility and consequences of choices. Ability to appraise living conditions to include physical and emotional environment. Ability to appraise the nature and the seriousness of threat to an elderly and/or disabled person. Ability to establish priorities according to relative importance and deadlines, set goals, and manage time efficiently. Ability to establish and maintain rapport with clients, co-workers, and contract agency staff. Ability to communicate with community members and advocates to request assistance for clients, promote support for the agency, and/or coordinate resources and programs. Ability to accurately complete work within required time frames. Education and/or experience with demonstrates the possession of knowledge, skills, and abilities for effective job performance. Ability to operate computer and general office equipment.

Initial Selection Criteria:
Prefer education and or experience indicating the individual possesses the knowledge, interviewing skills, and abilities necessary for job performance. Would prefer experience in interviewing the elderly and or disabled. Experience in using a computer and general office equipment. Preferred ability to communicate orally in English and Spanish. Must have reliable transportation. Must be able to conduct home/site visits in assigned geographical areas which may involve exposure to substandard and/or unsanitary living conditions. Must be able to attend training which may require overnight stay. Must be willing to travel up to 75% of the time. Occasional overtime may be required. This position is included in a career track series and may be filled at any level with the career track dependent on the selected applicant's qualifications as the relate to career track promotion eligibility requirement. If a former CCAD worker is selected as a worker within two years after holding that position, that applicant will be rehired at the classification level at which they left.
