HHHunt's Spring Arbor Senior Living of Williamsburg is seeking a qualified Cottage Care Coordinator. The Cottage is Spring Arbor's memory care community. The CCC plans, directs and assists as needed in all cottage care staffing and service needs. The CCC must have the ability to manage team processes and promote a team environment, as well as knowledge of medication and administration procedures and the social, psychological and recreational needs of Alzheimer’s and memory impaired residents. JOB RESPONSIBILITIES:
Manage

Recruit and hire cottage care staff
Manage and direct cottage care staff
Complete scheduling for cottage care staff
Orient, monitor progress and reinforce training of cottage care staff
Assist the Resident Care Coordinator and Executive Director in implementing state regulations as required
Complete all HHHunt and state required documentation in a timely manner
Function as part of the management team (manager on duty coverage, presenting the community, planning and participating in staff meetings, planning and implementing marketing and community events)
Resident Care Responsibilities

Develop a care team and residential living environment that supports overall Cottage Care philosophy
Create and coordinate implementation of ongoing person-centered activities and lifeskills programming
Complete resident assessments in a timely manner as required by HHHunt and state regulations
Coordinate resident care service needs in collaboration with care team and community department heads as appropriate
Monitor physician orders to assure follow-through and completion
Monitor all aspects of medication administration
Audit Medication Administration Records (MAR) and notify pharmacy of changes
Consult with families and/or physicians to coordinate resident care needs
Communicate with physicians and other healthcare professionals regarding medications, treatments and/or therapies

SUPERVISORY RESPONSIBILITIES:
Recruit, train, supervise, evaluate performance for and delegate responsibilities to cottage care staff.

KNOWLEDGE AND SKILLS:
Experience in and knowledge of diagnoses
Knowledge of medication and administration procedures
Knowledge of resources for seniors and of related agencies
Understanding of the social, psychological, and recreational needs of Alzheimer’s and memory impaired residents
Ability to exercise confidentiality following HIPAA guidelines
Pleasant, friendly, and helpful at all times ensuring proper and timely service
Genuine concern for and strong interest in working with seniors
Ability to demonstrate a positive attitude at all times
Professional appearance and demeanor
Dependable

QUALIFICATIONS:
Must be 21 years of age or older
Experience or training in an equivalent setting preferred
RN/LPN or Bachelor’s degree preferred
Med Tech certification required if not LPN/RN
General understanding of and concern for the needs of seniors
Aptitude and previous experience with Alzheimer’s and memory impaired residents
Ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to self or others
Ability to manage team processes and promote a team environment
