Part Time: Remote Patient Monitoring Representative (Telehealth) LPN

Join a growing, progressive organization dedicated to supporting parents and grandparents manage their health and safety as a Remote Patient Monitoring LPN! In this role, you will have the opportunity to help individuals with chronic conditions better manage their health by reviewing their daily readings submitted remotely, triaging for out of range results, and connecting them with medical care as needed. Schedule: Tuesday-Saturday 8:00am-12:00pm or Sunday-Thursday 9:00am-1:00pm

Monitor medical readings and respond as prompted by the system
Contact patients by phone to assess their needs
Confidently navigate technology, including monitoring systems and basic computer troubleshooting
Maintain HIPAA compliance
Provide exceptional customer service
Assist with clinical enrollment of new clients, support existing clients and collaboration between interdepartmental processes
Interact with customers, Case Managers, and Home Service Representatives’ in a professional, helpful and courteous manner
Provide basic, trouble-shooting technical assistance to customers as required
Other duties as assigned
LPN certification is Required
2- 5 years of related professional experience
Strong analytical and problem-solving skills
An energetic pace along with a positive attitude
Self-motivation and the ability to work independently and with teams
Proficient in the use of Word, Excel, Outlook, and PowerPoint
Excellent oral and written communication skills
Proficient in managing multiple tasks as the same time
Appropriate internet connection for remote work
