We are searching for a caring, dedicated and motivated Home Health Aide to join our Home Health and Private Duty team.

This individual must be comfortable working in client homes off campus, in Assisted Living and Independent Living locations. All Home Health personnel may serve clients in Johnson & Wyandotte counties.
DUTIES AND RESPONSIBILITIES
Assumes cheerful and positive attitude while providing personal care, rehabilitative and supportive services as assigned by the Registered Nurse or designee.
Engages in conversation that is neither personal nor controversial in nature.
Respects confidentiality of both patient and the Agency.
Assists in activities of daily living (ADL): personal hygiene, eating, dressing, elimination, exercise and ambulation.
Maintains, as much as possible, a safe environment. Gait belts shall be used when lifting, ambulating, and transferring patients.
Maintains, current and accurate clinical records.
Communicates changes in the physical and/or emotional status of the patient and/or family.
Provides household duties, depending up on need that may help facilitate the patient?s self-care at home. This may include: making bed, personal laundry, as well as other small household tasks.
Will participate in patient care conferences.
Develop and maintain a good working rapport with inter-department personnel and other departments within the facility, to assure that Home Health programs can be properly maintained to meet the needs of facility and residents.
Assure that the facility is maintained in a clean, safe and orderly manner for resident comfort and convenience.
Assure that work/assignment areas are clean and free of equipment, supplies, etc. before leaving such areas.
Ensure that resident?s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints are followed.
Attend and participate in in-service educational classes and on-the-job training programs.
All other duties as assigned by your supervisor or person in charge.
Has at least one (1) year experience as a Nursing Assistant within the past three (3) years.
Certification is preferred; four months is allowed for certification completion.
Previous Home Health Aide experience desirable.
Is physically capable of performing the job related duties.
Is able to provide dependable transportation as required by the nature of the position.
Must be able to be an approved driver in accordance with Lakeview Village requirements.
GED or High School Graduation Preferred.
Ability to working independently, display a kind, courteous attitude, and work in a team-oriented setting with residents, relatives, staff and others.
