Provides information and referral services and service coordination to families of children with special health care needs. Acts as a regional consultant to health care practices interested in aligning with best-practices in youth health transition. Champion and educator for families, community partners, and health care providers in the area of youth transition to adult healthcare. Coordinates work around the accomplishment of grant deliverables with particular focus on supporting the Northeast Regional Center for Children and Youth with Special Health Care Needs (www.northeastregionalcenter.org) serving 17 counties in NE Wisconsin.

Associate's or Bachelor's degree preferred.
A minimum of 2 years of related experience, preferably in healthcare, public health, social work or nonprofit agency.
Personal and/or professional experience working with or parenting a child with special healthcare needs preferred.
Strong interpersonal communication abilities, including effective public speaking experience and excellent writing skills.
Primarily first shift, but requires some evenings and weekends to support training and community outreach events.
Travel across NE Wisconsin and occasional statewide travel required.

Schedule: Tuesday - 8:00a m - 4:30 pm, Wednesday - 10:00 am -2:00 pm, Thursday - 8:00 am -4:30p m

  • Must have the ability to flex hours to support evening/weekend training or outreach opportunities on occasion
  • some regional travel across 17 counties in NE Wisconsin required
