The Division of Clinical Services is currently offering a new-hire sign-on bonus of $1,000 for this position located at our Headquarter's Office in Colorado Springs. The sign-on bonuse will be based on successful completion of the CDOC Training Academy (CTA) and successful performance reviews and evaluations.
Sign-On bonuses will be paid based on the following timeline:
$250.00 – Graduation of CTA
$250.00 – 6 Month Performance Review
$500.00 – 1 Year Performance Evaluation

Description of Job
This position coordinates medical care for offenders, primarily with high needs, who require chronic and/or long term care during incarceration or after Department release. This position is responsible for, but not limited to the following:
Implements the hospital discharge process by collaborating with the Third Party Administrator (TPA), infirmary personnel, Clinical Services Schedulers, Offender Services and others as needed to facilitate appropriate bed placement to meet the medical needs of the offender.
Serves as the Clinical Services liaison to the Offender Services unit to coordinate appropriate placement of offenders with medical needs.
Ensures that the appropriate Health Care Appliances (HCA) and medications are discharged with the offender upon release to the community.
Administrates Medicaid enrollment of offenders during incarceration.
Coordinates with Re-Entry and parole staff as needed on medical care needs and community placements of offenders needing skilled nursing, long term acute care, or other clinical settings.
Reviews offender housing restrictions as needed and communicates with facility providers to ensure restrictions are current.
Coordinates facility placement of offender care aides (OCAs) to ensure services are provided.
Monitors all offenders who enter the infirmaries for possible special needs supplies, HCA, or medications.
Collaborates with the infirmary staff to determine discharge plans to the permanent facility of those offenders who have been identified as having a special need.
Coordinates with the facility management staff, scheduling unit, offender services, and others to ensure accurate information is exchanged to facilities prior to discharge.
Coordinates and organizes the ordering and transportation of HCA or other special equipment or medications prior to the offender's discharge to a prison or community setting.
This position will be required to travel.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Applicant must possess a current, valid license as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
Applicant must have three (3) years of full-time experience providing patient care as a Registered Nurse.
A bachelor's degree from an accredited college or university in nursing may substitute for one year of the required experience. A copy of your official transcripts must be attached to utilize this substitution.
A master's degree or doctorate degree from an accredited college or university in nursing may substitute for two years of the required experience. A copy of your official transcripts must be attached to utilize this substitution.
Ability to follow patient confidentiality standards;
Ability to effectively treat offender patients;
Maintain required licensure/credentials;
Excellent interpersonal, written and oral communication skills;
Ability to read and comprehend the English language;
Ability to effectively communicate fluently in English both verbally and in writing;
Ability to proactively collaborate with internal and external stakeholders;
Ability to problem solve and think critically to effectively resolve issues;
Strong working knowledge of community resources used for discharge planning, hospital operations, third party payment systems and Medicare/Medicaid programs;
Ability to multi-task, set priorities and maintain organization;
Experience coordinating medical care as a Discharge Planner or Medical Case Manager in a hospital, institution, correctional or long-term care facility.
Experience working with the Medicaid application process.

Conditions of Employment

Successfully pass all modules of the Colorado Department of Corrections Basic Training, to include English reading comprehension, relevant competency assessments and written/practical exams in Defensive Tactics;
Possess the physical ability to successfully complete Defensive Tactics;
Lift arms above head and kick as high as own waist;
Stabilize another person to accomplish a controlled take down;
Use arms, palms of hands, shins, and feet to deliver blows;
Withstand impact (slow speed or with a safety bag) on own body from strikes/blows;
Swing a baton in a striking technique;
Rotate body 90 degrees with feet planted for striking with foot or using a defensive tactic;
Get down on one or both knees and up again with multiple repetitions;
Will be exposed to Oleoresin Capsicum during Basic Training and may be exposed while on assignment;
Must be able to see and hear in order to observe, address and respond to potential breaches in safety and security
Seize, hold, control or subdue violent or assaultive persons and defend oneself or others to prevent injury;
Demonstrate body agility, strength of all four limbs and the ability to walk and climb stairs consistently;
Work in situations involving assaultive behavior, physical control of another person and/or restraint situations;
Deal with individuals with a range of moods and behaviors, utilizing tactfulness and a congenial, personable manner;
Work with people of the opposite sex, diverse racial ethnic groups, mentally ill and emotionally disturbed offenders;

Willing and able to work various shifts, days, afternoons, graveyards (nights) which includes a rotating shift schedule;
TB (tuberculosis) screening is required of all new employees upon hire.
