Position Summary:
The Ambulatory Care Coordinator coordinates care across the healthcare continuum. Acts as a liaison between the patient, family/caregiver, physician, and other members of the healthcare team. Engages in multidisciplinary collaboration to ensure continuity of care and quality outcomes. Utilizes a patient-centered approach to assess for health-related needs, identify barriers, develop individualized care plans, and facilitate referrals to appropriate internal and community resources. Provides chronic disease-management to reduce utilization, prevent readmissions, and improve outcomes.
