What Your Role Will Be:

Support individuals to reach their personal goals, from daily tasks to personal achievements

Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community

Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives

Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals.

Support and promote the individual’s health and safety

Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.

Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping
Minimum Qualifications:
-
High School Diploma or GED required
-
Valid NYS Driver License with a minimum 1-year driving experience required
-
Experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion a plus
-
Experience with people with intellectual and developmental disabilities preferred

Must be able to operate Motor Vehicle
-
Evening and weekend shifts preferred
-
Must be able to lift up-to 50 lbs.
Work Location: Residential houses located in Nassau County and Suffolk
Work Hours/Schedule: Full Time, Part Time & Per Diem. Day, Evening & Overnight shifts available.
Pay: $18/hour

Paid Time Off:

Up to 80 hours of vacation during the first calendar year of employment. Employees are eligible to accrue 8 additional vacation hours each subsequent calendar year of employment up to a maximum of (120) hours
