Penn State Health Medical Group is seeking a patient-focused Manager, Care Management to lead our multidisciplinary team of professionals!

This is a high impact role which would be an excellent opportunity for an individual with ambulatory experience who is adept at leveraging resources in the community to benefit patients through the continuum of care.

The Manager, Care Management would be responsible for the overall development, implementation, and management of the Medical Group Case Management. This would include daily operations of case management as well as providing overall hiring, training, supervision and management responsibility for case management staff.

As a leader critical to ensuring the best outcomes for patients, this individual will evaluate and improve clinical performance by creating and maintaining a work environment that fosters quality care, patient satisfaction, interdisciplinary collaboration, team building and service excellence.

As a strategic leader, the Manager, Care Management will execute a strategic plan that includes an assessment of the service population’s needs, along with the capacity to serve those needs. This individual will also develop financial and operational targets/goals, monitor performance against established benchmarks, and prepare the annual capital and operational budget.

As people leader, this individual will supervise, coach, mentor and develop staff and support their professional development through continuing education and support of scholarly endeavors. This individual also serves as a role model and functions as a clinical resource for staff. As community liaison this individual will act as the champion to develop, coordinate, and participate in community awareness programs about diabetes/chronic disease prevention, diagnosis, and treatment.

MINIMUM QUALIFICATIONS:
Bachelor's Degree in Health related field; project management or business administration. 3 years ambulatory practice and or case management experience required.

PREFERRED QUALIFICATIONS:
Master's Degree preferred.
