Maxim Healthcare Services is seeking a RN Case Manager fora direct placement with naviHealth as a Skilled Inpatient CareCoordinator (SICC).

As a naviHealth Care Coordinator, you will facilitate andcoordinate the optimal Discharge Plan for each member in your assignedfacilities. You will utilize our nH Predict functional assessment tool as aguide to support efficiency and best outcome.

Patient and facility engagement including participation in IDT (interdisciplinary team meeting) and patient care conference
Utilization management of continued stay in SNF setting in compliance with CMS guidelines
Comprehensive coordination of Discharge Plan to support best outcomes and optimal LOS
Perform all SNF Concurrent reviews with appropriate application of medical necessity review criteria and decision support tools
Assure appropriate referrals are made to the Health Plan, and/or community-based services through involvement in IDT meetings and care conferences
Coordinate peer to peer and medical director reviews
Manage assigned caseload in an efficient and effective manner utilizing time management skills

Current RN license
Bachelor’s degree in Nursing preferred
Geriatric population experience preferred
5-8 years of advanced clinical experience
At least one year of Case Management experience Experience managing multi-disciplinary care teams, including nurses, therapists, social workers, case managers.
Proficient with Microsoft Office applications including Word, Excel and Power Point.
Strong presentation/public speaking skills
Current BLS card per contract requirements
Current TB or Chest x-ray (if applicable)
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required
Exceptional interpersonal and communication skills
Strong problem solving, conflict resolution and negotiating skills
Proficient with Microsoft Office applications including Word, Excel and Power Point
Independent problem identification/resolution and decision-making skills
Detail oriented
Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously
Team player
Ability to travel in a local or regional market depending upon facility alignment
Ability to establish a home office work space
Ability to function independently and autonomously with excellent self-discipline
Must be at least 18 years of age
