SOME is seeking an exceptional Youth Services Coordinator (Case Manager) within our Family Services program. This position shares with parents and families tools and education that will break the cycle of homelessness. A passion for working within homelessness, human services and mental health would be a great fit for this position. The brand new Conway Center is located directly across the street from the Benning Road Metro Station.

Position Description: The Youth Services Coordinator provides case management and support services for adults and children living in Family Housing and also, develops and coordinates programming for youth on life skills and education and personal development.

Qualifications:
Required: Bachelor's Degree; 2-3 years’ experience in social services; case management experience and program development experience

Preferred: Bachelor’s Degree; 3-5 years’ experience in social services; 2 + yrs. case management experience and program development experience; experience providing services to low-income families.

Required License/Certification: CPR/First-Aid, CPR for Infants, Driver's License and access to a vehicle.

Schedule: Monday-Friday, 3 days 11:30am-8p; 2 days 8am-4:30pm

Knowledge, Skills & Abilities Required:
Knowledge of family dynamics including of issues of trauma and mental health
Organizational, facilitation and teaching skills
Time management skills with the ability to multi-task despite competing priorities
Ability to build relationships and set professional boundaries
Ability to communicate with diverse audiences; both, verbal and written communication skills

Expected Contributions:
Case Management: The YSC will perform intakes and assessments on new residents. During the intake, the YSC will schedule times to meet with families and individuals regularly. Maintenance of progress notes and program charts starting with the intake must be at kept to standard. Also, will facilitate care and services with third party services providers.
Program Activity Development: The YSC will develop and coordinate activities in areas of improvement for residents. This can include life skills, personal development and/or professional development. These activities will require partnerships with outside resources. Events and activities will be planned quarterly to ensure maximum effectiveness.
